Overview
Conduit is the operating console for inter-provider referrals. The referring provider picks a patient from their roster, picks a specialist from the directory (filtered by specialty and accepting status), writes the reason and priority, and the referral enters a five-stage pipeline: sent → accepted → scheduled → completed → closed. The specialist's queue shows every incoming referral and exposes one-tap accept / decline (with reason) / schedule / complete. Completing a visit forces the specialist to draft a loop-closure report (impression + plan + medications-started + follow-up disposition), which sits in the referrer's inbox until they acknowledge it — only then does the referral reach the 'closed' stage. Both sides exchange threaded secure messages tied to the referral and attach documents (referral letter, prior labs, imaging) to the record. The referral-coordinator console aggregates live metrics: total, open, closed-this-month, loop-closure rate, average days to close, distribution by status / priority / specialty, plus a full audit feed. Designed for hospital networks and ACOs that lose money and quality scores to referral leakage.
